A report may show sample collection, issue and appointment dates. Keep those dates distinguishable so your saved history does not quietly substitute one event for another.
Preserve the event each date describes instead of choosing the most convenient number.
Start with the source
Read the labels on the original. A date beside “reported” describes a different event from a booked follow-up or the time an email arrived. If you cannot tell which date applies, keep the uncertainty in notes and check the source provider.
Make the review useful
Set the reviewed record date according to the document you are organizing. Include a useful explanation in the title or summary when several dates matter. A personal reminder date belongs to its action and should not overwrite historical metadata.
Keep the next step clear
When searching older records, compare the provider and patient label as well as the date. OneTap organizes the documents you supplied; a neatly ordered list does not prove every relevant clinical event is present.
Your little checklist.
Identify the labels attached to each visible date.
Choose the record date supported by the document.
Keep appointment and review dates in their own actions.
Check patient and provider context when returning to history.
A little reference.
Keep the context and your next action together.
| Check | Example | Your next step |
|---|---|---|
| Collection date | When a sample was taken | Keep its source label clear |
| Report date | When the document was issued | Use reviewed historical metadata |
| Review date | When you plan to read it | Schedule as a separate check-in |
Does importing the report today make today its report date?
No. Check the original document date; the time you import a source is not a substitute for the event it records.
